New Patient Registrationregister_now_blue_sign

If possible please bring your medical card to the surgery, along with two proofs of identity, one which must confirm your address and the other some form of photo identification.

If you don't have a medical card you will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms are available to download from the above "Registering Online" tab.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Please note that you are not obliged to attend the doctor whom you are registered; it is the policy of the practice to allow patients to choose whichever doctor in the partnership they wish to attend.

Medical treatment is available from the date of registration. Please contact reception for further information.
