PRESCRIPTION REQUESTS -Please use MHOL where possible for script requests. Patients will understand that the practice will now need to introduce temporary measures to reduce the risks to patients and staff.

If you need to order your repeat prescription this can be done by

  • either placing your request through the letterbox next to main entrance or prescription box situated in the entrance lobby. Please refrain from entering the main reception area. If you don't have your repeat prescription tear off slip, a request form can be accessed via our website or entrance lobby.
  • On MY HEALTH ON-LINE (MHOL) if registered to use

If you usually collect your prescription from the practice, we would ask patients to consider either providing a stamped self-address envelope or stating your preferred Pharmacy for collection in order to reduce the need to attend the practice.

Pregnancy Care Planner


The more you know about your pregnancy and your options, the more you are likely to feel in control. The information given here is based on The Pregnancy Book, which your midwife should give you at your first appointment.

New advice for pregnant women during Covid-19 outbreak- updated at 14.4.2020



In response to women and Community Midwives being unable to access the GP surgeries to complete a Maternity Booking Referral form we have set up an online system for women to inform the Community Midwives that they are pregnant. The email address will be checked regularly.

How does it work :

  1. Women send an email to the designated email address.
  2. They receive an automatic response with the referral form attached.

  3. They complete the form on –line and return it to the same email address.

  4. They will be contacted within a week by a Community Midwife who will then arrange a telephone booking appointment when they are 8-10 weeks pregnant.


This information is on the Cardiff and Vale UHB website and the Cardiff and Vale UHB Maternity Services Facebook page.

To ensure that the women are informed of the changes we would ask that you put some information on your website to this effect and possibly in the window of your surgery.

We would be grateful if you could let us know if women report problems with this system so we are able to see them within the recommended guidance.
